About the Role:

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at AccentCare. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Provide high-quality physical therapy services to patients in their homes, focusing on improving their mobility, strength, and overall well-being.
  • Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy.

What We Offer:

  • A competitive salary range of $115,000 - $125,000 per year, paid on a per-visit basis.

Requirements:

  • A Master's degree in physical therapy from an accredited program.
  • Current licensure to practice as a physical therapist in the state where you will be working.
  • At least nine months of experience as a physical therapist, with a preference for community or home health experience.
  • Valid CPR certification and a reliable vehicle for travel to patient locations.
