Job Description

We are seeking an experienced LCSW to join our team as a Rehabilitation Counselor - Home and Community. As a Rehabilitation Counselor, you will work with patients and their families to develop a personalized rehabilitation plan that addresses their unique needs and goals.

  • Develops and implements individualized treatment plans that promote maximum independence and quality of life for patients.
  • Provides counseling services to patients and their families to support their physical, mental, cognitive, and emotional adjustment through recovery.
Salary and Benefits

The estimated salary for this role is $80,000 - $110,000 per year. In addition to a competitive salary, we offer a range of benefits including:
