Ambulatory Care Pharmacist Position

We are seeking a highly skilled Ambulatory Clinical Pharmacist to join our Community Resource Center at Memorial Hermann Health System. This is an excellent opportunity for a motivated and detail-oriented individual to make a meaningful contribution to the health and well-being of our community.
Job Summary
As an Ambulatory Clinical Pharmacist, you will be responsible for providing healthcare support to underserved and uninsured populations in our community. Your primary focus will be on disease state management, medication therapy management, and education. You will work closely with our multidisciplinary team to develop, implement, and monitor clinical pharmacy programs that improve patient care and reduce healthcare costs.This is a full-time position with a Monday-to-Friday schedule from 9am to 5pm. No weekends or evening shifts are required. The ideal candidate will have relevant experience in ambulatory care pharmacy practice, residency training in pharmacotherapy or ambulatory care, and a strong passion for improving patient outcomes.
Key Responsibilities
- Develops and implements clinical pharmacy programs to improve patient care and reduce healthcare costs
- Provides medication therapy management services to patients and healthcare providers
- Collaborates with the multidisciplinary team to develop and implement disease state management programs
- Maintains accurate and up-to-date records of patient care activities and program evaluations
- Participates in quality improvement initiatives and contributes to the development of policies and procedures
Requirements
- Bachelor of Science degree with equivalent experience or clinically oriented Master of Science degree accepted; Doctorate of Pharmacy degree preferred
- Licensure in the State of Texas
- Pharmacy Sterile Product certification through an in-house program or other accredited program if appropriate
- One (1) year or greater experience as a Clinical Pharmacist I
- Ability to promote teamwork, build effective relationships, and to lead/facilitate work groups
Compensation and Benefits
The salary for this position is estimated to be between $120,000 and $150,000 per annum, depending on qualifications and experience.
