Key Responsibilities
  • Lead and model expected clinical behaviors with reference to our Care model, Signature Care
  • Oversee the team's QAPI Program and performance outcome metrics
  • Manage the Interdisciplinary Team (IDT)
  • Perform routine education on infection control, assessments, continuity of care, and compliance standards
  • Coordinate field visits with the nursing team and support patients and families as needed
  • Fulfill gaps with patient visits when necessary
  • Communicate the hospice philosophy, pain management, and the needs of the terminally ill to the clinical team, patients, and families
  • Demonstrate excellent interpersonal and communication skills
Requirements
  • Nursing Degree Holder: ADN, BSN, MSN
  • Licensed RN: Valid Texas RN License
  • Certified Professional: Valid BLS certification
  • Experienced Leader: Minimum of 2 years' experience in hospice care and previous Patient Care Manager experience preferred
  • Background Check Ready: Ability to pass background check and pre-employment drug screen
