Responsibilities
  • Collaborate with physicians and other healthcare professionals to provide comprehensive care to patients
  • Conduct physical exams, diagnose and treat illnesses, and prescribe medications as needed
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Five (5) years of experience in a continuity setting as a provider, required
  • Ability to work extended hours, including weekends, as needed
  • Strong communication and interpersonal skills, with a focus on patient-centered care
  • Ability to work collaboratively as part of a multidisciplinary team
