ROLE SUMMARY: The Dietitian at AHN is responsible for ensuring the nutritional health of our clients through the provision of medical nutrition therapy and health education services. The ideal candidate will have a solid background in conducting nutrition assessments and developing tailored nutrition plans. You will be expected to evaluate the effectiveness of interventions and utilize metrics for data-driven enhancements. Staying current with the latest nutrition research and integrating new findings into dietary recommendations is essential. The successful applicant will have experience in delivering nutrition education to patients and addressing risk factors. Collaboration with all members of a client's care team, including physicians, nurses, and caregivers, is crucial to customize dietary services and enhance patient outcomes. The Registered Dietitian should exhibit strong communication skills, critical thinking, and leadership qualities.

ESSENTIAL QUALIFICATIONS:

  • Bilingual (English/Spanish) preferred.
  • Must successfully pass a background check.
  • Valid driver's license required for driving company vehicles.
  • Comply with HIPAA regulations regarding protected health information.
  • Adhere to all AHN policies and guidelines.

KEY DUTIES AND RESPONSIBILITIES:

  • Evaluate client needs and collaborate with the team to create individualized education plans that promote skill development for effective disease management.
  • Identify specific disease self-management and medical nutrition therapy goals, developing tailored educational care plans to meet individual needs.
  • Implement personalized, culturally appropriate nutrition strategies for diverse populations.
  • Communicate with referring providers regarding patient care plans and progress, establishing baseline data for ongoing evaluation.
  • Facilitate health education for patients and families on disease self-management, addressing topics such as healthy eating, physical activity, and medication management.
  • Conduct individual counseling sessions for clients focusing on disease self-care skills and risk factor reduction.
  • Build and maintain positive relationships with AHN program partners through diligence and timely follow-up.
  • Work with clients and care teams to identify barriers to achieving positive health outcomes.
  • Screen for and refer clients to appropriate health-related programs and classes.
  • Advocate for clients with program partners and healthcare providers to ensure access to necessary resources.
  • Participate in team meetings to enhance individual and program effectiveness.
  • Ensure patient records are current and complete with necessary documentation.
  • Maintain accurate records and documentation in the database system.
  • Manage a diverse caseload, including individuals living with chronic conditions.
  • Interact respectfully with clients while fostering a collaborative team environment.
  • Exhibit compassion and professionalism in serving our population.
  • Engage effectively with patients to promote desired behavior changes and health outcomes.
  • Complete required training in relevant topics such as motivational interviewing and chronic disease management.
  • Be willing to see clients in various healthcare settings as needed.
  • Demonstrate flexibility in work hours based on departmental needs.
  • Maintain a positive attitude and sense of humor.
  • Perform other related tasks as assigned.

EDUCATIONAL REQUIREMENTS:

  • Bachelor's degree in a related field from an accredited institution.
  • Current Registered Dietitian credential required.

EXPERIENCE REQUIREMENTS:

  • Minimum of one year of professional experience as a registered dietitian.
  • Experience providing medical nutrition therapy to at-risk populations.
  • Experience in delivering disease self-management education.
  • Experience working with socioeconomically disadvantaged groups.
  • Proficiency in Microsoft Office applications.
  • Strong interpersonal and customer service skills.

WORK ENVIRONMENT:

  • Moderate noise level in a climate-controlled office environment.
  • Ability to stand for 4-6 hours and sit at a computer for up to 6 hours daily.
  • Ability to lift and carry up to 30 pounds regularly.
