Key Responsibilities:
The selected candidate will be responsible for:
- Conducting thorough patient examinations and gathering comprehensive medical histories and physical examination results.
- Diagnosing and managing common medical conditions while consulting with physicians for chronic illness management.
- Performing various therapeutic procedures, including injections, immunizations, suturing, wound care, and infection management.
- Creating and executing patient management plans, documenting progress notes, and ensuring continuity of care.
- Assisting in the review of medical charts for patient follow-ups and quality assurance purposes.
- Providing guidance and counseling to patients on adherence to treatment plans, developmental milestones, family planning, emotional health, and preventive care.
- Maintaining comprehensive medical records for all patients, including follow-up referrals, and regularly collaborating with physicians on charting and medical care.
- Meeting productivity standards set by the Chief Medical Officer and senior management.
- Engaging in quality improvement initiatives to enhance patient care.

Qualifications:
The ideal candidate will possess the following qualifications and skills:
- A minimum of 3 years of clinical experience as an FNP or PA, preferably in an ambulatory care setting.
- A valid California medical license, including a DEA license and current CPR certification.
- Bilingual proficiency in Spanish and English is preferred.
- Local candidates or those willing to relocate are encouraged to apply.
- Experience with electronic medical records (EMR) is required; training will be provided, and familiarity with EPIC is a plus.
- A strong commitment to community-based healthcare.
