Primary Care Physician Specialist

CareOregon is a nonprofit organization dedicated to providing medical care exclusively in the home in the Western United States. We are seeking a Primary Care Physician Specialist to join our team.
About the RoleThis position provides medical care and treatment services with an emphasis on chronic disease management and promotion of health and palliative care for patients not best served in a clinic setting. The ideal candidate will have experience in medical home visits, geriatric and/or palliative care certified, and knowledge of urgent care and triage practices for providers focused on relief care.
Responsibilities- Provide medical consultation and/or support to other practitioners within the organization.
- Diagnose and treat acute and chronic medical conditions according to scope of practice.
- Develop and implement proactive patient-centered care plans with the assistance of other disciplines within the care team.
- Educate patients and families on goal-based, appropriate preventative care, chronic care or urgent care needs, as well as the use of prescribed medications and treatment.
- Licensed physician (MD or DO) in the State of Oregon.
- Completion of internship and residency in Internal Medicine or Family Medicine.
- Certification by the American Board of Medical Specialties.
- Drug Enforcement Administration (DEA) license.
- Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
We offer a competitive salary range of $198,280 - $242,340 per year, 5% bonus target, and full benefits. This is a fully benefitted 0.8 FTE position serving East Portland.
