Key Responsibilities
1. Deliver personalized care in accordance with each resident's individualized Plan of Care.
2. Monitor and assess changes in residents' health status, adjusting care plans as necessary.
3. Exhibit strong assessment and documentation skills to ensure accurate record-keeping.
4. Collaborate with healthcare teams to develop and refine the Resident Plan of Care.
5. Maintain compliance with facility policies while providing care to residents.
6. Administer primary skin care treatments under the guidance of attending physicians and nursing leadership.
7. Identify and manage various skin conditions, including ulcers and lesions.
8. Ensure residents at risk for skin breakdown receive appropriate preventive care and treatment.
9. Engage in the creation and execution of tailored care plans alongside multidisciplinary teams.
10. Implement comprehensive nursing care through thorough assessment, planning, and evaluation.
11. Administer medications and treatments as prescribed, continuously evaluating their effectiveness.
12. Foster a supportive environment that promotes the overall well-being of residents.
13. Participate in educational initiatives for residents and their families.
14. Supervise and mentor team members, contributing to their professional development.
15. Delegate nursing responsibilities to team members, ensuring completion to high standards.

19. Act as a resource for staff, addressing inquiries and resolving issues related to resident care.

22. Ensure availability of necessary medical supplies for wound care procedures.
23. Conduct wound assessments, including dressing changes and treatment applications.
24. Maintain accurate daily and weekly documentation of care provided.
25. Assess new admissions and readmissions to establish care needs.
26. Communicate effectively with healthcare providers, families, and nursing leadership regarding resident care.
