**Salary: Up to $95,000 per year**

The estimated salary for this position ranges from $85,000 to $95,000 per year, reflecting the cost of labor across several U.S. geographic markets.

**Key Responsibilities**

You will coordinate the admissions process for hospice patients by making skilled assessments of their initial physical, psychosocial, and spiritual needs.

Establishing the initial plan of care with the attending physician, medical director, and the interdisciplinary team is a crucial part of this role.

You will provide skilled interventions and instructions to patient and caregivers to ensure optimum symptom control and patient comfort under the direction of physician.

Functioning as a liaison between disciplines and facilities during the admission process is also essential.

Maintaining individualized patient care progress records with accuracy, timeliness, and according to policies is critical to this role.

**Qualifications and Benefits**

To succeed in this role, you should have a current RN license, one year of clinical RN experience, and current CPR certification.
