We are seeking a dedicated and compassionate Integration Specialist to join our team at Sea Mar Community Health Centers. As an Integration Specialist, you will play a vital role in providing health home services and support to patients with complex medical, behavioral health, and social concerns.

This is a full-time position available for the Care Management department in Anacortes, WA. The ideal candidate will have a strong understanding of community outreach or case management and be able to work effectively with an interdisciplinary care team.

The estimated salary for this position is $26.52 per hour, making it a competitive opportunity for professionals in the field. As an Integration Specialist, your primary responsibilities will include:

  • Providing health home services and support to patients with complex medical, behavioral health, and social concerns
  • Conducting timely and effective screenings and referrals to internal Sea Mar service providers, community-based resources, and emergency services when indicated
  • Developing and implementing individualized care plans with patients to address their unique needs and goals
  • Collaborating with healthcare providers, caregivers, and family members to ensure seamless coordination of care
