Key Responsibilities
  • Conduct comprehensive assessments of patients to identify treatment needs and develop individualized treatment plans.
  • Collaborate with multidisciplinary teams to provide case coordination and management services.
  • Develop and implement discharge plans to ensure a smooth transition for patients.
  • Communicate effectively with patients, families, and community providers to ensure functional communication of needs.
Requirements
  • Bachelor's degree in social work, education, or a related field.
  • One year of experience in mental health care social work preferred.
  • Knowledge of principles of social work, human behavior, and contemporary social and economic conditions.
  • Ability to analyze, assemble, and organize information.
