The Montana State Hospital is seeking a skilled Behavioral Health Care Planner to join our team. As a Behavioral Health Care Planner, you will provide assessment, treatment planning, and supportive individual follow-up to individuals with serious mental illness.
Key Responsibilities- Conduct comprehensive assessments of patients to identify treatment needs and develop individualized treatment plans.
- Collaborate with multidisciplinary teams to provide case coordination and management services.
- Develop and implement discharge plans to ensure a smooth transition for patients.
- Communicate effectively with patients, families, and community providers to ensure functional communication of needs.
- Bachelor's degree in social work, education, or a related field.
- One year of experience in mental health care social work preferred.
- Knowledge of principles of social work, human behavior, and contemporary social and economic conditions.
- Ability to analyze, assemble, and organize information.
The Montana State Hospital provides inpatient psychiatric treatment for adults with serious mental illness. We are committed to serving Montanans in their communities to improve and protect their health, safety, and well-being.
