Job Summary

Key Responsibilities
  • Provide physical therapy services to patients, including evaluations, treatments, and progress monitoring.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Conduct initial assessments, form treatment plans, and communicate effectively with patients and medical staff.
  • Complete reports, assessments, and progress notes using approved forms in patients' records.
  • Be reachable for phone consultation during duty hours.
  • Maintain rigorous daily charting.
Requirements
  • Valid and active license issued by the Physical Therapy Board of California.
  • At least 12 consecutive months of performing similar services within the last 3 years.
  • CPR and BLS certification from American Heart Association (AHA) or Red Cross (with AED).
  • Must be proficient in the English Language (Speaking/Writing).
Work Environment

This position is located in a correctional facility in Delano, California. The ideal candidate will be able to work in a fast-paced environment and provide high-quality care to patients in a correctional setting.
