We are seeking a skilled Nurse Practitioner to join our team in Mohave County, AZ. As a key member of our Home and Community Care team, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes.

Your Key Responsibilities:

  • Conduct comprehensive in-home assessments, including evaluations of past medical history, medication reconciliation, and vital signs.
  • Perform evidence-based practice screenings and identify diagnoses for active medical management.
  • Communicate findings with patients' primary care physicians to inform care management.
  • Educate patients on disease processes, medications, and compliance.
  • Travel up to 85% to provide home visit assessments.

Requirements:

  • An active NP or PA license or ability to obtain by start date.
  • Active national NP or PA certification or ability to obtain national certification in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
  • Access to reliable transportation for home visit assessments.

Preferred Qualifications:

  • Additional licensure in other geographic areas.
  • 1+ years of clinical experience in their highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
