As a key member of our team, you will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.
Key Responsibilities- Deliver and Coordinate Quality Patient Care: Provide skilled nursing care to patients in their homes, ensuring compliance with physician orders and developed care plans.
- Assess and Document Patient Condition: Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Participate in Care Planning: Participate in the development, implementation, and update of the nursing care plan, ensuring patient needs are met and outcomes are achieved.
- Provide Education and Support: Educate patients and families regarding disease processes, self-care techniques, and prevention strategies, promoting patient autonomy and independence.
- Maintain Competencies: Participate in educational programs, continued education units, and skill evaluations to maintain knowledge and competencies related to the nursing profession.
- Graduate of an Accredited School of Nursing: Must be a graduate of an accredited school of nursing.
- Current, Unrestricted State License: Must hold a current, unrestricted state license as a Licensed Nurse in the state of practice.
- CPR Certification: Must hold current CPR certification.
- Experience and Skills: Must have one year of experience working under a current nursing license, with a strong focus on clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Recent Experience as a Licensed Nurse: Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home Health Experience: Experience working in home health care settings, with a strong understanding of patient needs and care requirements.
- English Language Proficiency: Must be able to speak, write, read, and understand English.
- Travel and Mobility: Must be able to travel and have the ability to lift 50 pounds, with prolonged walking, standing, bending, kneeling, reaching, and twisting.
- Visual and Hearing Acuity: Must have strong visual and hearing acuity, with a strong sense of smell and touch.
