Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Sapphire Nursing at Meadow Hill, a long-term skilled nursing facility in Newburgh, New York.
Job Summary:The successful candidate will be responsible for evaluating and treating patients/residents, communicating with their families, physicians, and other health team members, and maintaining all necessary documentation of services in the patient's medical records.
Key Responsibilities:- Evaluate residents' conditions and develop appropriate physical rehabilitation plans.
- Help residents improve their mobility and relieve their pain using exercises, hands-on therapy, and stretching.
- Teach residents how to use physical therapy equipment and help them through their tasks and monitor their performance.
- Maintain residents' confidence and protect the facility by keeping information confidential.
- Comply with federal, state, and local legal and professional requirements.
- Contribute to team effort and submit all billing logs and necessary information on a timely basis.
- Graduate of an accredited Physical Therapy Program.
- Current New York State PT License.
- Nursing Home Experience preferred.
- Full Benefits Package.
