Job Summary

Northwestern Medicine is seeking a skilled Physical Therapist to join our Outpatient In-house Registry team. As a key member of our rehabilitation services team, you will be responsible for developing and implementing physical therapy programs aimed at maximizing function while educating patients regarding their limitations.

The ideal candidate will have a Bachelor of Science in Physical Therapy and be licensed to practice in the State of Illinois. A strong understanding of regulatory specifications and policies is also essential. Key Responsibilities
  • Develop and implement physical therapy programs to maximize patient function
  • Evaluate patients and gather information to determine accurate clinical diagnoses and treatment plans
  • Communicate treatment programs to patients, families, and interdisciplinary personnel
  • Coordinate and implement treatment plans, modifying as needed
  • Treat patients using various techniques, including therapeutic exercise, functional activities, and manual techniques
  • Evaluate home, work, and community settings and make recommendations for safe and efficient function
  • Re-evaluate patients at appropriate timeframes and document goal progression
  • Plan and communicate discharge needs to patients, families, and medical personnel
  • Maintain accurate and timely documentation in accordance with department policy
Qualifications
  • Bachelor of Science in Physical Therapy
  • Current State of Illinois Physical Therapy license
  • BLS certification through the American Heart Association
