Job Title: Licensed Practical Nurse in Home Health

Citizens Memorial Hospital (CMH) is seeking a skilled Licensed Practical Nurse (LPN) to join their dynamic team providing services to patients in their homes.

A typical day as an LPN will include:

  • Developing a relationship with patients and their supportive healthcare team
  • Providing excellent care to patients, utilizing strong critical thinking and assessment skills, in accordance with federal and state regulations

The LPN will see 5-6 patients to provide skills in the home.

Job Requirements:

  • Associates degree in Nursing
  • Patient care experience
  • LPN/LVN license
