We are seeking a skilled social worker to provide direct services to patients and families in a medical setting. This is a great opportunity for a motivated and experienced professional to join our team and make a meaningful difference in the lives of our patients.
Responsibilities
- Provide direct social work services to patients and families identified as having complex needs
- Work closely with healthcare teams to develop and implement care plans
- Collaborate with patients and families to identify and address social determinants of health
- Participate in multidisciplinary teams to improve patient outcomes
Requirements
- 2+ years of recent experience as a social worker in a medical setting
- Graduate of a school of social work accredited by the Council on Social Work Education
- Current LSW or LISW license in Ohio
- Bachelor's degree or master's degree (preferred) in social work
