Job Summary:

We are seeking a Per Diem Social Worker to provide social work services to our clients in Jersey City, New Jersey. The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide social work services to clients in their homes, including assessments, goal setting, and interventions.
  • Develop and implement individualized care plans that meet the unique needs of each client.
  • Collaborate with the multidisciplinary team to ensure comprehensive care and support for clients.
  • Provide education and support to clients and their families on various topics, including coping strategies, stress management, and community resources.
  • Document all interactions with clients and families in a timely and accurate manner.
Requirements:
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
  • At least 2 years of experience in social work, preferably in a home health care setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
