Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Wexford Health Sources in Decatur, IL.

Job Summary:

The Licensed Practical Nurse will provide high-quality patient care in a correctional facility setting. This role will involve assisting in the delivery of patient care through assessment, planning, implementation, and evaluation. The successful candidate will work under the direct supervision of a designated Wexford Health manager and clinical supervision from the Director of Nursing, Registered Nurses, Medical Director, and/or IDOC personnel as appropriate.

Responsibilities:
  • Assist in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
  • Direct and guide patient teaching activities commensurate with education and demonstrated competencies.
  • Perform non-professional nursing care duties in the assessment of the physical, psychological, and social dimensions of patients.
  • Assist in planning individual treatment programs by utilizing available resources in planning care and consults with registered nurses, physicians, and correctional officers.
  • Implement individual treatment programs in the Health Care Unit and, as necessary, in the housing unit.
  • Implement clinical and technical aspects of care in accordance with established policies and procedures.
  • Document nursing encounters, patient response, effectiveness, complications, etc., and communicates information to supervisory personnel and physicians, in accordance with physician orders.
  • Observe, record, and report symptoms, reactions, and changes in the condition of all patients.
  • Use available in-house resource personnel and departments for treatment/consultation.
  • Carry out designated nursing procedures.
  • Respect the dignity and confidentiality of patients.
  • Take vital signs (temperature, pulse, respiration, and blood pressure).
  • Apply and change dressings.
  • Provide for emotional and physical comfort and safety of patients.
  • Assist with activities of daily living with special attention to therapeutic significance.
  • Assist in maintaining working and living areas and supplies in good order.
  • Serve as a member of a nursing care and treatment team.
  • Assist with orientation of new employees.
  • Assist physician as assigned.
  • Supervise inmates in the maintenance, cleanliness, and sanitation of the environment.
  • Administer medications and injections.
  • Assess personal and learning development needs.
  • Plan and implement programs to meet identified needs through in-service attendance and other formats and informal means.
  • Demonstrate knowledge of, and perform nursing policies and procedures.
  • Support the philosophy, objectives, and goals of the nursing department.
  • Perform within the normal limits of the State Nursing Practice Act, Code of Ethics for Nurses, policies and procedures, and facility policy/procedures and standards.
  • Maintain the confidentiality of inmate records.
  • Approach change in a positive manner.
  • Make suggestions for improving work conditions to the Medical Director.
  • Accept constructive criticism in stressful situations.
  • Perform other duties as required or assigned.
Requirements:
  • Currently licensed as an LPN in the state of Illinois.
  • Current CPR certification.
  • Graduate from an accredited School of Nursing.
  • One (1) year clinical experience preferred.
Work Environment:

The Licensed Practical Nurse will work in a standard medical setting in a correctional facility, which includes daily interfacing with inmates, correctional staff, and health care personnel. Temperature extremes may be encountered while moving throughout the facility outdoors and indoors in buildings without air conditioning.
