Job Summary

Coastline is seeking a skilled Occupational Therapist to join our team on a per-diem basis. As an Occupational Therapist, you will play a vital role in providing personalized care services to our clients.

Key Responsibilities
  • Conduct initial assessments to determine the scope and types of personal care services required.
  • Develop and implement personalized care plans to meet the unique needs of each client.
  • Collaborate with our team to ensure seamless delivery of care services.
  • Maintain accurate and up-to-date documentation as required by the Mass Health PCAM Program.
  • Evaluate patients' eligibility for the Mass Health PCA Program using the standard Mass Health evaluation tool.
Requirements
  • Current licensure by the Massachusetts Division of Registration in Allied Health Professions and in good standing.
  • Current certification by the National Board for Certification in Occupational Therapy in good standing.
  • Valid MA Driver's License with a dependable vehicle.
  • Excellent organizational and interpersonal skills, with a working knowledge of community resources and reimbursement systems for health and social services.
  • Ability to communicate effectively within a fast-paced team environment.
  • Self-motivated, with the ability to work independently.
  • Bilingual preferred, but not required.
