About the Role

We are looking for a skilled and experienced RN to provide high-quality care to our patients in the North Houston area. As a Hospice RN Case Manager, you will be responsible for developing and implementing individualized care plans, coordinating with the interdisciplinary team, and providing emotional support to patients and their families.

Responsibilities

Requirements
