The Sigma Healthcare team is seeking an experienced Physical Therapist to work with our contracted facility. As a Physical Therapist, you will be responsible for initiating physical therapy programs and instructing other personnel and/or family/caregivers in certain phases of physical therapy.
Responsibilities:
Provides physical therapy services to patients according to a written physician's plan of care.
Assesses and evaluates therapeutic/rehabilitative/functional status and participation in the development of the total plan of care.
Directs physical therapy treatment and instructs patients/caregivers in the use and care of therapeutic appliances.
Determines priority needs for physical therapy and reports to the physician patient's reaction to treatment or changes in condition.
Requirements:
BLS/BCLS certifications
Valid Drivers License
Valid state license (Where applicable)
New Grad are okay
SNF Experience preferred
