Key Responsibilities for Physical Therapists:
- Collaborate with physicians to create tailored care and treatment plans suitable for the client's home environment, focusing on fall risk and safety measures.
- Evaluate muscle strength, mobility, gait, range of motion, and pain levels, selecting appropriate treatments and assistive devices while instructing family members on their use.
- Document client responses to treatments and any changes in their condition to the Director of Nursing and the physician.
- Initiate and guide physical therapy programs, training other personnel, including family members, in specific phases of care.
- Prepare clinical summaries and participate in discharge planning, submitting reports to the Director of Nursing.

Key Responsibilities for Occupational Therapists:
- Conduct comprehensive physical and psychological assessments of clients and develop or follow treatment plans.
- Assess home and work environments to determine necessary adjustments for clients.
- Recommend adaptive equipment to assist clients with daily activities.
- Create rehabilitation programs to help clients regain lost skills.
- Educate caregivers and family members on patient care and the use of adaptive equipment.
- Monitor and evaluate the effectiveness of occupational therapy interventions.
- Stay informed about advancements in professional knowledge and treatment practices.

Qualifications for Physical Therapists:
- Master's degree or higher in physical therapy from an accredited program.
- Successful completion of the National Physical Therapy Examination (NPTE) and/or state examination.
- Valid state licensure for the location of service.
- Minimum of two years of relevant experience, preferably in community or home health settings.

Qualifications for Occupational Therapists:
- Bachelor's degree in occupational therapy from an ACOTE or AOTA accredited university.
- Certification from the National Board for Certification.
- Compliance with state healthcare regulations.
- Up-to-date knowledge of treatment practices and rehabilitation programs.
- Strong interpersonal communication skills and the ability to work with diverse client personalities.
