About This Role

As a Licensed Practical Nurse with our organization, you will play a vital part in delivering exceptional patient care in the comfort of their own homes. Under the guidance of a Registered Nurse, you will coordinate patient care plans, assessing, planning, and implementing nursing interventions to meet individual needs.

Your Responsibilities
  • Care for patients in various settings, including homes and hospitals
  • Collaborate with healthcare teams to maintain high standards of professional practice
  • Contribute to creating personalized care plans tailored to each patient's requirements
What You'll Need to Succeed
  • A current License Practical Nurse license within New York State
  • Strong communication and interpersonal skills to work effectively with patients and healthcare professionals
  • The ability to assess patient needs and develop appropriate care plans
