About the RoleAs a KVNA Physical Therapist, you will work with patients to improve their movement and manage their pain, ensuring they can function independently and maintain their dignity.
You will be responsible for assessing and evaluating patients' therapeutic, rehabilitative, and functional status, developing individualized plans of care, and instructing patients and their families/caregivers on the use and care of therapeutic appliances.
You will also report to physicians on patients' responses to treatment and changes in condition, identify patients' and families' needs for other home health services, and refer as necessary.
Key Responsibilities- Assess and evaluate patients' therapeutic, rehabilitative, and functional status
- Develop individualized plans of care and instruct patients and their families/caregivers
- Report to physicians on patients' responses to treatment and changes in condition
- Identify patients' and families' needs for other home health services and refer as necessary
- Communicate with physicians and other practitioners related to patients' current home health plans of care
- Participate in discharge planning for patients
- Supervise Physical Therapist Assistants according to policy and procedure and state regulations
- Submit thorough clinical notes and progress summaries based on achievement of goals
- Participate in the agency's quality assessment and performance improvement program
- Possess a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization
- Licensed to practice as a physical therapist in the State of Wisconsin
- Two years of appropriate experience as a physical therapist; community or home health experience is preferred
- Licensed driver with an insured automobile in good working order
- Current CPR certification
