Primary Care Physician

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at Oak Street Health. Our ideal candidate is passionate about delivering high-quality, patient-centered care to older adults in a community-based setting.
About the Role:
- Provide exceptional primary care services to patients at our local clinics, including assessments, diagnoses, and treatment plans.
- Oversee and direct the primary care team, ensuring seamless delivery of care.
- Prescribe and administer pharmaceutical treatments and medications as needed.
- Maintain accurate and up-to-date Patient Electronic Medical Record data via Canopy and Greenway systems.
- Collaborate with regional and central leadership to meet health quality goals and drive positive outcomes.
- Work closely with Practice Managers to direct and manage the center care team, ensuring efficient operations and effective communication.
- Other duties as assigned by management.
Requirements:
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
- Internal Medicine or Family Medicine Board Certification (or board eligible).
- Active, non-probationary, unrestricted State License.
- Active DEA license.
- US work authorization.
