Responsibilities:
- Develop and implement personalized care plans in collaboration with physicians and healthcare teams.
- Provide emotional support and intervention to patients and their families during times of crisis.
- Conduct thorough assessments of patients' social, emotional, and economic needs, and develop strategies to address these factors.
- Connect patients with vital community resources and services to ensure they have access to everything they need to thrive.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Qualifications:
- Masters degree in Social Work from an accredited institution.
- Current licensure to practice as a social worker in the state of employment.
- At least one year of experience in a healthcare setting, preferably in home care or a related field.
- CPR certification.
