Job Summary

Mass General Brigham is seeking a skilled Per Diem Clinical Social Worker to join our team. As a Per Diem Clinical Social Worker, you will be responsible for providing high-quality clinical services to patients and their families, while also collaborating with multidisciplinary teams to ensure seamless care transitions.

Key Responsibilities
  • Assessment and Treatment Planning: Conduct comprehensive assessments of patients' needs and develop individualized treatment plans to address their clinical and psychosocial needs.
  • Collaboration with Multidisciplinary Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care and effective communication.
  • Discharge Planning: Develop and implement discharge plans that address patients' ongoing needs and ensure a smooth transition to community-based care.
  • Documentation and Communication: Maintain accurate and timely documentation of patient interactions and communicate effectively with patients, families, and healthcare teams.
Requirements
  • Education: Master's in Social Work (MSW) required.
  • License: LICSW in the state of MA (or equivalent independent Social Work licensure in state where services are being provided).
  • Experience: A minimum of two years post-degree experience or equivalent in an inpatient, partial, or outpatient setting.
Preferred Qualifications
  • Clinical Skills: Broad range of clinical skills, including psychiatric assessment, crisis stabilization, family support, and intervention.
  • Communication Skills: Excellent written and verbal communication skills, with ability to work collaboratively with multidisciplinary teams.
  • Computer Skills: Familiarity with computerized clinical databases and ability to use electronic systems for communication and administrative purposes.
