About the Role

At CHAS Health, we are seeking a highly motivated and detail-oriented Patient Resources Coordinator to join our team. This is an exciting opportunity to make a meaningful impact in the lives of our patients and communities.

Key Responsibilities
  • Work with a diverse patient population, providing education and support to ensure they receive the healthcare services they need.
  • Review patient demographics to determine eligibility for various health insurance programs and community resources.
  • Enroll patients in Medicaid, CHIP, and other health insurance programs, as well as assist with applications for assistance programs such as food assistance and childcare assistance.
  • Develop and maintain relationships with community partners to ensure seamless access to healthcare services.
  • Provide training and support to staff on current insurance, managed care, and resource-related issues.
  • Track enrollments, managed care conversions, and other activities on a regular basis.
Requirements
  • High school diploma or equivalent required; health industry experience preferred.
  • Excellent verbal and written communication skills, with the ability to work effectively with diverse populations.
  • Basic computer and typing skills required; proficiency in Microsoft Office and other software applications preferred.
  • Valid driver's license and insurance required.
