About the Role

Amedisys, a leading home health and hospice company, is seeking a skilled Hospice Liaison to join their team. This role requires experience in outside sales/business development in the healthcare industry, with a focus on generating leads and building relationships with key referral sources.

Responsibilities
  • Educate healthcare professionals, patients, and families about home care services.
  • Develop and maintain relationships with hospitals, nursing homes, and physician offices to generate leads.
  • Coordinate patient admissions with the team, including obtaining required documentation.
  • Conduct on-site visits to assess patient needs and identify barriers to care.
  • Keep accurate records of patient interactions and referrals.
  • Achieve admissions, product, and payor-mix goals.
  • Meet short and long-term target account goals.
  • Document sales calls daily in CRM, including pre and post-call notes.
  • Maintain a complete record of targeted referral sources in CRM.
  • Attend community events to promote Amedisys.
  • Respond to customer complaints in a timely manner.
Requirements
  • At least one year of experience in a healthcare-related business development/outreach role.
  • Reliable transportation, current driver's license, and liability insurance.
