Job Summary

Emerson Hospital is seeking a skilled Physical Therapist to join our Home Care team. As a Physical Therapist in this role, you will be responsible for providing comprehensive patient management, including evaluation, diagnosis, and intervention, as well as patient and family education. This will involve developing and implementing individualized care plans in the home setting.

Key Responsibilities:

  • Reporting to the Rehabilitation Team Lead, you will work in a multidisciplinary fashion with specialty care groups, including Parkinson's Disease, Covid Recovery, and Post-Acute Stroke care.
  • You will have the opportunity to develop and implement individualized care plans, working directly with patients and their families to achieve optimal outcomes.
  • As a member of our Home Care team, you will have the flexibility to work in a family-friendly environment with flexible hours.

Requirements:

  • Education: Bachelor's Degree or higher from an accredited physical therapy program.
  • Experience: Prior Home Care experience is preferred, or at least one year of experience in an acute and/or rehab setting.
  • Licensure and/or Certification: MA PT license and BLS required within 30 days from hire.
  • Skills: Clinical expertise for patient population, ability to prioritize workload and respond to urgent needs, communication skills, assessment skills, critical thinking skills, organizational skills, problem-solving skills, computer skills, ability to maintain confidential information, and ability to read, write, and communicate in English.
