About the Role:
The MDS Coordinator RN is responsible for developing, completing, and transmitting resident assessments (MDS - Minimum Data Set) in compliance with current Federal and State regulations. This role requires a thorough understanding of MDS processes, utilization reviews, and Quality Improvement initiatives.

Key Responsibilities:

  • Establish and manage the MDS schedule, coordinating with the Interdisciplinary Team (IDT) to ensure timely completion.
  • Complete MDS assessments according to the RAI manual, ensuring accuracy and timely submission.
  • Transmit finalized MDS to CMS and analyze validation reports to address any discrepancies.
  • Review hospital records to determine appropriate MDS Assessment Reference Dates (ARD) and identify principal diagnoses for skilled coverage.
  • Lead PPS meetings to ensure skilled residents receive appropriate care and coverage.
  • Verify compliance with Medicare billing documentation and ensure all necessary forms are completed promptly.
  • Supervise LVN MDS Nurses and engage in quality improvement discussions with the leadership team.
  • Participate in ongoing education and training to stay updated on RAI guidelines and best practices.
