Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Homecare Team in Columbia/Greene Counties. As a RN, you will provide high-quality care to patients and their families in the comfort of their own homes.

Responsibilities
  • Develop and implement comprehensive care plans in collaboration with the interdisciplinary team.
  • Provide direct patient care, including medication management, wound care, and symptom management.
  • Conduct regular assessments and evaluations to ensure patient needs are met.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices in hospice care.
Requirements
  • Current license to practice as a Registered Nurse in the State of New York.
  • Associates Degree in Nursing, BSN preferred.
  • At least one year of experience as an RN, preferably in hospice/home care or acute care.
  • Valid Driver's License and reliable vehicle.
  • Excellent communication and interpersonal skills.
