- Conduct home visits, assessments, and investigations to identify and address potential risks and needs.
- Document all interactions, assessments, and services provided in a timely and accurate manner.
- Bachelor's or Master's degree in a human services field, such as social work, sociology, psychology, or a related field.
- At least 30 semester credits in human services areas, such as social work, sociology, psychology, or a related field.
- Experience working with vulnerable populations, including adults with disabilities, mental health conditions, or other challenges.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and community agencies.
