Job Summary

Mission Neighborhood Centers, Inc. is seeking a highly motivated and experienced Case Manager to join our team. As a Case Manager, you will provide intensive 1-1 wrap-around case management services to at-risk youth in our programming, including crisis intervention, counseling, advocacy, mentorship, and information referrals.

Key Responsibilities
  • Provide case management services to youth and families, including assessment, goal setting, and service planning.
  • Develop and implement individualized service plans to address the unique needs of each youth.
  • Collaborate with community agencies and organizations to provide resources and referrals to youth and families.
  • Provide crisis intervention and counseling services to youth and families as needed.
  • Participate in case conferences and court sessions to advocate for youth and families.
  • Conduct home visits and other outreach activities to engage with youth and families.
  • Maintain accurate and up-to-date records of youth and family interactions, including case notes and service plans.
  • Participate in regular team meetings and training sessions to stay current on best practices and program developments.
Requirements
  • Bachelor's degree in Social Work, Counseling, or related field.
  • 2+ years of experience in case management or a related field.
  • Knowledge of trauma-informed, restorative, and harm reduction practices.
  • Ability to work with diverse populations, including youth and families from diverse cultural and linguistic backgrounds.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
