Alternate Solutions Health Network LLC is seeking a skilled Home Health Occupational Therapist to join our team in Canton, Ohio.
Job Summary:We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in their homes. As a Home Health Occupational Therapist, you will work collaboratively with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.
Responsibilities:- Conduct comprehensive assessments of patients' physical and cognitive abilities to identify areas for improvement.
- Develop and implement individualized plans of care that address patients' goals and needs.
- Provide occupational therapy services to patients in their homes, including exercises, activities, and education on proper use of equipment.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
- Document patient progress and outcomes in accordance with agency policies and regulations.
- Current licensure as an Occupational Therapist in the state of Ohio.
- Minimum one year of experience as an Occupational Therapist in a home health setting.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team.
