Key Responsibilities- Provide Collaborative Primary Care: Work closely with our primary care team to deliver comprehensive, patient-centered care to our members.
- Execute Longevity Health Model of Care: Develop and implement individualized plans of care for our members, focusing on treating in place and using clinical practice guidelines to guide treatment decisions.
- Facilitate Health Plan Clinical Operations: Administer health plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
- Serve as a Trusted Advisor: Educate care teams on member needs and early identification of changes in condition, notifying APP and telehealth partners for member changes.
- Deliver Effective Communication: Communicate effectively with members, loved ones, and care team members on member care needs, goals of care, treatment plans, and changes.
- Support Market Goals: Balance member caseload demands with other team member support needs, promoting overall market goals and targets.
- Promote Longevity Health: Engage with members, loved ones, and nursing facilities to promote Longevity Health and its services.
- Function Autonomously: Work independently, demonstrating critical thinking skills, organization, and prioritization skills.
- Credentials: State unrestricted licensed Advanced Practice Nurse or Physician's Assistant, National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner or NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
- DEA License: Required.
- Active National Provider Number: Must have an active national provider number (NPI) and an active Medicare provider number.
- Eligibility to Participate in Medicare: Must be eligible to participate in the Medicare program as a provider and registered.
- Valid State Driver's License: Valid state driver's license with a good driving record and proof of automobile insurance required.
- Master of Science in Nursing: Master of Science in Nursing as a Nurse Practitioner from an nationally accredited program or Master's Degree from an nationally accredited program.
- Minimum of 6 Months Experience: Minimum of 6 months experience as a Nurse Practitioner/Physician's Assistant and some experience with geriatric/hospice or palliative care population prior to becoming an NP/PA.
- Experience in Nursing Home Environment: Experience working in the nursing home environment is strongly desired.
- Computer/Technology Proficiency: Computer/technology proficiency required.
- Excellent People Skills: Ability to engage and communicate effectively with physicians, patients, family members.
- Self-Directed but Functions Well in a Team Environment: Self-directed but functions well in a team environment.
- Excellent Clinical Skills: Excellent clinical skills - assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
