Compensation and Benefits
- $45.00 - $63.00 per visit/unit
- $70,500 - $96,900 per year base pay

Responsibilities
- Coordinate care and services for patients, including scheduling appointments and ordering supplies.
- Develop and implement individualized care plans, ensuring patients receive the best possible care.
- Provide education and support to patients and their families, promoting healthy behaviors and disease prevention.
- Monitor patients' progress and adjust care plans as needed.
- Collaborate with the interdisciplinary team to ensure seamless care coordination.
Requirements
- Diploma, Associate or Bachelor Degree in Nursing
- EMR Home Care Home Base Preferred
- Home Health experience Required
- OASIS proficiency Required
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation
