The System Float Pool Case Manager is responsible for facilitating comprehensive care plans for patients on designated units and/or service lines across the Houston Methodist hospital system. This role requires collaboration with physicians and interprofessional healthcare teams to ensure high-quality, efficient, and cost-effective care.
Key Responsibilities
- Review medical necessity of admission, continued stay, and resource use
- Identify areas for process improvement and drive change through evidence-based practice and performance improvement projects
- Collaborate with external case managers to facilitate discharge planning and communicate with payors to secure reimbursement
Qualifications
- RN licensure in Texas State or Compact State
- Two years of experience in case management
- Strong assessment, organizational, and problem-solving skills
