Hillcrest Health Services is seeking a skilled and compassionate Hospice LPN to join our Nebraska Hospice team. As a member of our interdisciplinary team, you will provide holistic, patient-centered care to individuals in need of end-of-life support.
Key Responsibilities- Assess and manage patient care needs, including symptom management and crisis intervention.
- Collaborate with physicians, family members, and other caregivers to develop and implement individualized care plans.
- Provide nursing services in accordance with established standards and protocols.
- Participate in service line meetings and team conferences to ensure coordination of patient services and prioritization of needs.
- LPN licensure in the state of Iowa or compact state required.
- Three years' experience as a nurse, with prior hospice or home health care experience preferred.
- Valid state driver's license, current automobile insurance, and reliable transportation.
- Average to above-average computer skills and ability to work independently.
