Amedisys is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Hospice Care. As a key member of our care team, you will be responsible for providing patient-centered care and collaborating with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Develops, prepares, and maintains accurate and timely patient care progress records.
- Teaches patients, families, and caregivers on patient care techniques and provides education on pain and symptom management.
- Regularly communicates with clinical managers and care team members to ensure seamless care coordination.
- Performs on-call responsibilities and provides on-call services to patients and families as assigned.
- One year of nursing experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
