Job Title: Nurse Practitioner

Join CenterWell Senior Primary Care, a leading provider of senior-focused primary care, as a Nurse Practitioner in our field-based team. We are seeking a highly skilled and compassionate healthcare professional to provide proactive, preventive care to seniors in the Alpharetta, Cumming, and Cherokee areas of Georgia.

About the Role:

As a Nurse Practitioner with CenterWell, you will work independently to achieve optimal patient outcomes, applying advanced education and clinical competencies to diagnose and treat acute and chronic conditions. You will provide health promotion and maintenance through examinations, diagnoses, and treatment plans, as well as prescribe medication and coordinate care with other healthcare professionals.

Responsibilities:
  • Provide high-quality, patient-centered care to seniors in their homes, focusing on preventive care, chronic condition management, and minor injury treatment.
  • Conduct thorough examinations, diagnose conditions, and develop treatment plans in collaboration with patients and their families.
  • Prescribe medication and coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
  • Work closely with our integrated care teams to address the physical, emotional, and social well-being of our patients.
  • Stay up-to-date with the latest clinical guidelines and best practices in geriatric care, ensuring that our patients receive the highest level of care.
Requirements:
  • Valid state driver's license and clean driving record.
  • Active, unrestricted state license, DEA license, and board certification.
  • Minimum 2 years of clinical experience, preferably in geriatric care or primary care.
  • Strong technical skills, including proficiency in electronic medical records (EMRs) and HIPAA-secure messaging apps.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams.
