Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their physical, emotional, and spiritual needs. Develop and implement individualized care plans to address these needs and promote optimal patient outcomes.
- Implementation and Evaluation: Implement care plans and interventions to address patient needs. Evaluate the effectiveness of these interventions and make adjustments as needed to ensure optimal patient outcomes.
- Collaboration and Communication: Collaborate with other healthcare professionals to provide comprehensive patient care. Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Education and Support: Provide education and support to patients and families to promote health literacy and self-management. Develop and implement individualized education plans to address patient learning needs.
- Documentation and Reporting: Accurately and thoroughly document patient information in the electronic medical record. Prepare and submit reports to healthcare teams and other stakeholders as needed.
- Education: Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred.
- License: Current license to practice as a Registered Professional Nurse in New York State.
- Experience: Minimum one to two years experience in a hospital setting. Critical care experience required.
- Certifications: BLS, ACLS, PALS required.
