We are seeking a highly skilled and compassionate Exceptional Family Member Program (EFMP) Case Liaison to join our team at Commander, Navy Installations. This is a unique opportunity to make a meaningful impact in the lives of military families with special needs.

About the Role

As an EFMP Case Liaison, you will serve as the primary point of contact for families enrolled in the program, providing support and guidance to help them navigate the complexities of accessing community resources and services. You will work closely with healthcare professionals, commands, and community organizations to ensure seamless coordination and effective delivery of care.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build trusting relationships with diverse individuals and families. A strong background in case management, social sciences, or a related field is essential, along with knowledge of DoD and Navy policies and procedures.

Key Responsibilities
  • Provide comprehensive support to EFMP families, including assessment of their special needs and situational stressors, identification of available resources, and development of personalized plans to improve quality of life.
  • Collaborate with healthcare professionals, commands, and community organizations to ensure effective delivery of care and timely access to resources.
  • Stay up-to-date with relevant policies, procedures, and best practices in the field of case management and social sciences.
  • Develop and maintain accurate records and reports to track progress and identify areas for improvement.
Requirements
  • A degree in behavioral or social science or a related field is preferred, or a combination of education and experience that demonstrates equivalent knowledge.
  • At least one year of experience working in a case management capacity with families who have members with special needs.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with diverse individuals and families.
  • Knowledge of DoD and Navy policies and procedures, including the Exceptional Family Member Program.
