Key Responsibilities:
- Provide high-quality physical therapy services to patients in their homes, focusing on improving their mobility, strength, and overall well-being.
- Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Communicate effectively with patients, families, and caregivers to educate them on treatment plans, progress, and goals.
Requirements:
- Master's degree in physical therapy from an accredited program.
- Current licensure to practice as a physical therapist in the state of operation.
- Minimum nine months of experience as a physical therapist, with a preference for community/home health experience.
- Valid CPR certification and a reliable vehicle for travel to patient locations.
Benefits:
- Competitive salary range: $102,000 - $160,000 per year.
