About the Role: In-Home Primary Care Provider
Embrace the flexibility of working from home or directly engaging with members in their communities as an In-Home Primary Care Provider with Easy Health. You'll be at the forefront of delivering preventative care and educational services, leveraging local travel opportunities and face-to-face interactions. This position offers a unique balance between work-life fulfillment and local travel convenience.
Key Responsibilities:
- Perform comprehensive assessments, such as Annual Wellness & Quality Gap Care visits, for our members in their homes.
- Develop and maintain relationships with members, ensuring their needs are met and providing tailored support.
- Utilize proprietary tools and electronic medical records to accurately document clinical findings and track patient progress.
Requirements:
- Current unrestricted license as a Nurse Practitioner in the state(s) where services are rendered.
- Board-certified, preferably in geriatric or adult medicine specialty.
- Familiarity with the HCC model, quality HEDIS measures, and STAR metrics.
- Effective communication skills, particularly when interacting with elderly and chronically ill members.
- Proficiency in using computer, tablet, and mobile technology for documentation and communication.
- Willingness to travel within designated geographic areas.
