Position Overview
Under the guidance of the Registered Dietitian Manager, you will utilize your extensive knowledge of clinical nutrition and specialized diets to conduct nutritional assessments and reassessments for participants. You will gather food preferences, monitor adherence to dietary modifications, evaluate current diets and supplements, conduct meal rounds, and ensure sanitation standards are met. Additionally, you will provide nutrition education to participants, caregivers, and our health staff.
Key Responsibilities
- Collaborate as a vital member of the interdisciplinary team (IDT); perform initial, semi-annual, unscheduled, and annual assessments of participants; develop and implement personalized nutritional care plans, including physician orders for special diets.
- Engage in the creation and execution of Quality Assurance and Performance Improvement (QAPI) initiatives.
- Supervise dietary aides and conduct regular meetings to ensure effective communication and support.
- Monitor therapeutic diets and special dietary modifications, addressing any concerns with the interdisciplinary team; assess the need for liquid nutritional supplements and order as necessary.
- Track participant food intake and follow up with dietary aides, the interdisciplinary team, and participants regarding any comments or concerns.
- Evaluate participants' access to food and refer them to the interdisciplinary team for meal preparation evaluations, Meals on Wheels, sack lunches, grocery shopping assistance, and meal preparation services.
- Communicate with in-home service providers about participants' meal preparation skills, kitchen safety, grocery stock, and food and fluid intake at home.
- Ensure the adequacy of lunch and snack menus for the center.
- Update necessary documentation such as sack lunch lists, supplement sheets, and feeding room guides.
- Assist with meal service and monitor inventory in refrigeration, freezers, and dry storage.
- Maintain regular communication with vendors.
- Oversee dietary expenditures, prepare budget reports, anticipate future budgetary needs, and discuss them with your supervisor.
- Ensure a safe and sanitary working environment; adhere to safety policies and procedures; handle potentially infectious materials with appropriate precautions.
- Provide basic nutrition and sanitation education for participants, families, primary caregivers, and staff as needed.
- Participate in wound care meetings, family meetings, and departmental meetings.
- Responsible for staff recruitment, orientation, training, and performance evaluations; set clear expectations and delegate appropriate responsibilities to staff; hold staff accountable for their duties and adherence to company policies.
- Engage courteously and positively with participants, families, staff, and external partners, demonstrating effective partnership management skills.
- Maintain confidentiality regarding all company procedures, results, and participant information.
- Attend and actively participate in staff meetings, training sessions, projects, and committees as assigned.
- Adhere to the center's practices, procedures, and policies, including assigned break times and attendance.
- Accept assigned duties cooperatively and perform other related tasks as needed.
- Be adaptable in your work schedule.

- Must be able to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds, using proper body mechanics.
- Corrected vision and hearing within normal range, with or without reasonable accommodation, is required.
- Must document care provided in participant service records.
- Ability to work under stress; conditions may be noisy and crowded with fluctuating indoor temperatures.
- Exposure to potential risks of bodily injury through contact with moving equipment, toxic substances, and communicable diseases.
Experience
Minimum of one (1) year of documented experience working with frail or elderly populations.
Knowledge, Skills, and Abilities
Comprehensive knowledge of clinical nutrition and therapeutic diets; ability to assess clients' nutritional needs; skilled in medical terminology and documentation.
Education and Certification
Bachelor's degree from an accredited institution in food science, nutrition, or dietetics. Certification by the Commission on Dietetic Registration (CDR) as a Registered Dietitian is required. Must be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct participant contact.
