As a Licensed Practical Nurse (LPN) at University Hospitals Avon Rehabilitation Hospital, you will play a vital role in the interdisciplinary team providing care to patients who require continued rehabilitation to recover completely. Our hospital is dedicated to delivering superior patient care and serving our community with a strong mission.
Responsibilities- Administer medications accurately and safely, considering the age and clinical condition of the patient.
- Provide patient care that meets the standards of care for geriatric and general inpatient rehabilitation patients.
- Assess patients and reassess as per policy, identifying and initiating appropriate rehabilitation nursing interventions under the supervision of the R.N.
- Perform timely and accurate QI assessments under the supervision of the R.N.
- Develop and implement individualized plans of care based on patient needs and response to treatment, evaluating and updating the plan daily.
- Interact professionally with patients and families, involving them in the formation of the plan of care.
- Assess and manage pain effectively, educating patients and families on pain management techniques.
- Graduate of an accredited licensed practical/vocational nurse program.
- Current state licensure as a Licensed Practical/Vocational Nurse.
- CPR/BCLS certification, ACLS preferred.
- IV Certified or obtained within 6 months highly preferred.
- Minimum six months Medical/Surgical experience in an acute care setting preferred.
- Excellent oral and written communication and interpersonal skills.
